Senior Connection Center’s Long Term Care Services Unit is here to help individuals and families navigate long-term care options with confidence. Our knowledgeable specialists can:

• Connect clients with appropriate long-term care services and supports in our area
• Assess long-term care needs for waitlist placement
• Answer questions about long-term care options
• Provide counseling on available services
• Make referrals within or outside the agency as needed
• Assist, when appropriate, with the Pre-Enrollment List and Medicaid eligibility process for the Statewide Medicaid Managed Care Long Term Care Program

Important Update for Professionals:

We have launched a new referral form for social workers, home health agencies, and healthcare professionals. This form is now the best way for professionals to submit referrals for an over-the-phone assessment for Long Term Care Services.

If you are referring a client, please complete the new Client Referral Form for Professionals.

Individuals and families may still:

• Call 1-800-96-ELDER (1-800-963-5337) to speak with an Elder Helpline Specialist
• Call 1-800-336-2226, then choose option 3 or 4 to be screened for Medicaid Long Term Care Services
