If you are a caregiver supporting a loved one who is age 60 or older and you reside in Hillsborough, Hardee, Highlands, Manatee or Polk County, TCARE is here for you!
Caregiving in any form can be both a joy and a sacrifice, and can lead to caregiver burnout. When you struggle with caregiving responsibilities, it’s not only extremely hard on you, but it is also is difficult for the person you’re helping. Fortunately, Senior Connection Center has a program designed to prevent caregiver burnout.
The Tailored Caregiver Assessment and Referral System Program (TCARE) is an accredited program that offers customized support to family caregivers.
What TCARE Does
- Creates a care plan just for you that offers support and improves your effectiveness as a caregiver
- Reduces your stress and provides strategies for reducing root causes of caregiver burnout
- Connects you with resources for your physical health and mental well-being
- Promotes aging in place and may delay nursing home placement for your loved one
- Please note that TCARE does not provide funding for in-home services or paid caregiver services. However, if this is something you are interested in, you may inquire about this with the TCARE Support Specialist, and she can make a referral to our Long Term Care Services Department for further assistance. Alternatively, you may call the Senior Connection Center at 1-800-336-2226 or 813-740-3888 and press option 2 to learn more about in-home and paid caregiver services offered through our Long Term Care Services Department.
How TCARE Works
Once you complete your TCARE application, Senior Connection Center’s TCARE specialist will meet with you twice over the phone. In the first meeting, the specialist will confirm your information and schedule a screening. Your specialist will also create a care plan just for you, which you will discuss during the second meeting. Moving forward, the specialist will reach out to you monthly to check on you and see how the plan is working.
